Children and Young Person’s ADHD NHS (Right to Choose) Services
Our service model is designed to support the best possible clinical outcomes. Our assessment pathway is tailored specifically for children and young people aged 6 to 17½ years. If a young person is due to turn eighteen within the same calendar year, it is often more appropriate for them to wait and be referred directly into adult services. This ensures continuity of care and avoids a transition partway through the assessment process.
To maintain an efficient and high-quality service, we kindly ask that all questionnaires are completed within the specified timeframes. Providing this information promptly enables us to progress your child’s referral without interruption. If forms are submitted late, this may result in a delay to the assessment process.
For the ADHD service, there are two key stages — Pre-assessment and Assessment:
Pre-assessment
This stage gathers initial information to determine whether an assessment is appropriate.

Initial referral for ADHD assessment (by healthcare professional).

Patient completes pre-assessment questionnaires.

Multidisciplinary Team (MDT) reviews case notes and questionnaires to confirm next steps.

Step 1: Getting Started
You will receive a secure link to begin your journey.

- Provide all required information
- Share school contact details
Complete everything as soon as possible to avoid delays.
Step 2: Questionnaires
You MUST have an informant to continue.
✔ A teacher, SENCO, or school staff member
✔ A valid email address for them
Without this, your assessment will be delayed.
Why do we need an informant?
Clinicians need to understand how your child behaves in different settings.
- Build a full developmental picture
- Understand day-to-day functioning
- Ensure accurate diagnosis
This must be completed before your assessment can progress.
Step 3: Clinical Triage
Important:
There may be a delay at this stage whilst we review if an assessment is suitable at this time, please be aware we may reach out to request additional information. If you don’t not hear from us for over 2 weeks please reach out.
Step 4: Part A - Online Assessment
Step 5: Part B - In-Person/Remote Assessment
A face-to-face or remote session lasting approximately 90 minutes.
Step 6: Report & Support
Please note:
- The school report will need to be completed prior to moving forward with the referral. Parents or carers are encouraged to action this asap and prior to any planned school holidays, as this can extend the processing times.
- The same carer should be present throughout the process, if possible, to help with the best possible care of the patient.
- Please have a list of current medications ready for each appointment.
If you’re registered with a GP in England, you may be eligible to access our services for free through the NHS Right to Choose scheme. This pathway allows your GP to refer you directly to HealthHarmonie Minds, avoiding long NHS wait times while still receiving fully NHS-funded care.
